Position Summary:

We are seeking a dedicated Registered Nurse (RN) - Case Manager for a contract position to provide nursing case management services during the prenatal and postpartum periods, including infant care. This role includes screenings, education, interventions, and home visits to ensure client progress and well-being.

Key Responsibilities:

• Provide nursing case management for prenatal and postpartum clients.

• Screen for Social Determinants of Health (SDOH), perinatal depression, and risk factors.

• Educate clients on perinatal care standards and provide referrals.

• Develop and track individualized care plans.

• Complete postpartum home visits within 30 days of delivery.

• Respond to communications within one business day and document all interactions in the EMR.

Qualifications:

• Valid RN license (Maryland or Compact State Agreement).

• CPR Certification.

• Proficient in Microsoft Office and EMR (Epic preferred).

• Strong communication skills.
