Join Our Team as a Registered Nurse Case Manager for HIV/AIDS program.
Location: Hyattsville MD
Hours: 8:00 AM - 4:30 PM; Full-Time (40 hours/week) Contract position.
What You’ll Do:
- Develop and manage personalized care plans for clients.
- Coordinate multidisciplinary care and ensure access to vital services.
- Educate clients and families on HIV health and treatment options.
- Maintain quality standards for all active cases.
What We’re Looking For:
- Current Maryland RN License.
- CPR Certification.
- At least 1 year of related experience.
- Strong communication skills and proficiency in Microsoft Office and EMR systems.
